an improved randomizer for names in the shopping ditto

My students love it when I include their names in math dittos.   I like the shopping ditto format because I can use it at different times of the year:  when learning + & - decimals, when learning X decimals, and when learning %'s.  However, when I enter my class list into the program, it randomly chooses names for the problems.  And, it truly is random - in the mathematical sense.  What that means in effect, though, is that I may have one student named 3 times, another twice and several students not named at all. 

When I create a worksheet with 20 problems, I usually have only 9 - 13 students (out of 20 in my class) named in problems.  The other problems are all multiples of students already named.  I have even tried to see what would happen if I reduced my class size to less than 10 students and created a worksheet with 20 questions ... still some students with no problems and others with 2 or 3!  I'd love to see a randomized program that will also ensure that multiples are reduced/eliminated until all students in the class list have been chosen at least once.

It would also be nice if this worksheet allowed for tax rates with decimals instead of just whole numbers.  (i.e. a tax rate of 7.5% or 7.25%).
